Give your page a reasonable title (like \u201cService Agreement\u201d) and paste the content of your service agreement into the body.<\/p>\n<p>Where do you get your service agreement content? Ideally, you want an attorney to put this information together for you to ensure you are protected from your customers and the law. You can write your own agreement, but there\u2019s a chance you\u2019ll leave something out or implement a policy that leaves you vulnerable to lawsuits.<\/p>\n<p>That said, there are a few resources online to help you create service agreements without hiring an attorney. Don&#8217;t be afraid to use design elements like columns, images, charts, and clever typography to make your page easier to understand. Furthermore, use simple language wherever possible. This will increase the likelihood that your readers understand the page and follow your policies.<\/p>\n<p>Consider adding a navigation table of contents on the page (using anchor links). This will help visitors find specific pieces of information they need. For example, instead of scrolling through the page looking for shipping information, they could just click the \u201cShipping Policies\u201d link.<\/p>\n<p>Once your service agreement contract is in place, save the page as a draft.<\/p>\n<h2>Create Your Service Agreement Form<\/h2>\n<p>Your next step is to add a form to the page to collect digital signatures. This way you won\u2019t have to send a blank contract and receive a signed one. Give the form a unique name, such as \u201cService Agreement Form.\u201d If you need help creating new forms, <a href=\"https:\/\/www.gravityforms.com\/create-form-wordpress\/\">read our full guide<\/a>.<\/p>\n<p>Unlike a <a href=\"https:\/\/www.gravityforms.com\/job-application-form-wordpress\/\">job application form<\/a>, <a href=\"https:\/\/www.gravityforms.com\/heres-how-to-accept-donations-through-your-wordpress-site\/\">donation form<\/a>, or <a href=\"https:\/\/www.gravityforms.com\/registration-forms\/\">registration form<\/a>, a service agreement form is quite simple. You don\u2019t need to collect a lot of information. In fact, you only need enough information to identify the submitter\u2019s signature and contact them if there\u2019s an issue.<\/p>\n<p>Add the following fields to your form:<\/p>\n<ul>\n<li>Name<\/li>\n<li>Date<\/li>\n<li>Email<\/li>\n<li>Phone<\/li>\n<\/ul>\n<p>You\u2019ll find all of these under <strong>Advanced Fields<\/strong>.<\/p>\n<p><img loading=\"lazy\" class=\"alignnone size-full wp-image-13442\" src=\"https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/advanced-fields.png\" alt=\"Service agreement form\" width=\"308\" height=\"488\" srcset=\"https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/advanced-fields.png 308w, https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/advanced-fields-189x300.png 189w\" sizes=\"(max-width: 308px) 100vw, 308px\" \/><\/p>\n<p>You may also want to include a checkbox field (under <strong>Standard Fields<\/strong>) with only one option. This is where they\u2019ll check that they understand and agree to the terms. Yours might look something like this:<\/p>\n<p><img loading=\"lazy\" class=\"alignnone size-full wp-image-13441\" src=\"https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/acknowledgement-check-box.png\" alt=\"Service agreement form\" width=\"519\" height=\"673\" srcset=\"https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/acknowledgement-check-box.png 519w, https:\/\/www.gravityforms.com\/wp-content\/uploads\/2019\/04\/acknowledgement-check-box-231x300.png 231w\" sizes=\"(max-width: 519px) 100vw, 519px\" \/><\/p>\n<p>Next you\u2019ll need to add the signature field. This is the most important component of the Service Agreement.<\/p>\n<p>If you installed and activated the Signature Add-On in step two, you should see the signature field under <strong>Advanced Fields<\/strong>. Simply drag that field to the bottom of your form.<\/p>\n<p>Play around the field settings until you have it set up to your liking. You can change the color, size, background, border, and style under Appearances.<\/p>\n<p>Make sure you check the \u201crequired\u201d box on every field. Then save the form.<\/p>\n<h3>Step 4: Adjust Your Confirmations and Notifications<\/h3>\n<p>In the form editor is a <strong>Settings<\/strong> drop down. Under that menu are options for <strong>Confirmations<\/strong> and <strong>Notifications<\/strong>.<\/p>\n<p>Confirmations are on-screen messages to let the submitter know the form was successfully submitted. The default option is suitable for most people, but you may want to customize the wording a bit.<\/p>\n<p>Notifications are emails sent to you, the submitter, or anyone else whenever a form is submitted. By default a Gravity Form comes with a single notification sent to the WordPress administrator. You may want to add an additional email address to that notification to make someone else on your team aware. Or you may want to create an additional notification to send a copy to the submitter.<\/p>\n<p>You can use <a href=\"https:\/\/docs.www.gravityforms.com\/merge-tags\/\">merge tags<\/a> to dynamically populate submitted field values and other dynamic information in notification emails.<\/p>\n<p>The {all_fields} tag will display all of the form\u2019s fields except the digital signature field. We left it out for security reasons. If you want to include the digital signature field in an email, use the {signature} merge tag.<\/p>\n<h2>Embed the Form on Your Service Agreement Page<\/h2>\n<p>Your final step is to embed your service agreement form on your service agreement page.<\/p>\n<p>The easiest way to embed the form is by using the <a href=\"https:\/\/www.gravityforms.com\/gutenberg-gravity-forms\/\">Gravity Forms Gutenberg Block<\/a>. This adds a new block to your Gutenberg editor (the new WordPress editor) specifically for Gravity Forms. <a href=\"https:\/\/docs.www.gravityforms.com\/gravity-forms-gutenberg-block\/\">Read our documentation<\/a> to learn how to install this block.<\/p>\n<p>Once the Gravity Forms Gutenberg Block is installed, simply drag it to the place on your page where you want the form to appear. Some options will appear on the right side of the editor. Use the Shortcode Block to open a field to insert the shortcode. Change # to the form number.
